Health and safety reporting form

The Isle of Man Government is committed to ensuring the health, safety and welfare of all its employees.

In order to ensure that the health and welfare of all employees and safe environments are maintained, it is important that when incidents do occur, that they are reported to the Health and Safety Section using the Health and Safety Reporting Form.

Examples of such incidents are listed below

• Near Miss
• Minor Injury
• Minor Injuries to members of the public
• Serious Injury or Occupational Disease
• Specified Dangerous occurrence
